If you are admitted to a hospital, nursing home or rehabilitation center fourteen (14) days or fewer before an election, you may arrange to have a ballot delivered to you at the hospital. You and your doctor must complete and sign a legal affidavit that serves as an application form and arrange to have it delivered to the Cook County Clerk’s Office.
