In order to participate in this five year program a resident must fill out an application form, is a registered voter, and have a permanent disability. Cook County will then automatically send an absentee ballot application within those five years.
Applications for this program can be downloaded at the Cook County Clerk’s website or call the Cook County Clerk’s Office at 312-603-0938. Applications are also available at the Village Hall.
