How to Obtain Fire and Emergency Medical Services Records/Reports

The South Chicago Heights Fire Department shall protect the people and property within our Village. The South Chicago Heights Fire Department is responsive to the needs of our citizens by providing rapid, professional, humanitarian services essential to the health, safety, and well-being of the city. The South Chicago Heights Fire Department will achieve our mission through prevention, education, fire suppression, emergency medical, and other emergency services. We will actively participate in our community, serve as role models, and strive to effectively and efficiently utilize all of the necessary resources at our command to provide a quality of service deemed excellent by our citizens with “Courage, Integrity, and Honor.”
HOW TO OBTAIN OPEN RECORDS
The Chief of the South Chicago Heights Fire Department serves as the Administrator of the Department’s public records. Records typically requested are environmental site assessments, and emergency medical or incident reports.

Please include a way to be contacted to ensure a reply. There is a $25 fee for EMS report requests. Please include payment with your request to avoid any delays in processing. An invoice can be generated by emailing cvlietstra@southchicagoheights.com.
All requests will be processed as soon as practicable and without delay.
